Preparing The School Portrait
Package Production Form
1. Indicate your customer number.
2. Assign each school its own unique job number. This job number will be used for any absentee, make-up or retake dates at this school.
3. List the school enrollment. The enrollment allows us to plan for staffing and materials neccessary to produce your job.
4. Assign every job a name.
5. Please indicate the number of rolls or disks you are sending in.
6. Circle the appropriate grade level and job schedule.
7. Check the appropriate boxes for makeups and reshoots. Check “1” if the job is the original day, or “2” if the job is a makeup, etc. If you are submitting images for the final shoot, check this box.
8. For film capture, indicate if you have sent camera cards with your order. Camera cards are required with any computer or digital product such as name labels, composites, rolodex cards, CDs, pictorial directories, barcode labels, etc.
9. For film capture, how are you indicating edits? (Check the appropriate boxes)
10. For digital capture, how do you supply images? Indicate whether you have pre-edited your images and how you supply data/order info. If you prefer, Excel Photo will prepare data from camera cards. To have us perform that service, choose “Other” and specify “Camera Cards”.
11. Indicate the number assigned to any predefined package structures. This number will be given to you by customer service when you send order envelopes or flyers prior to the first scheduled photo date. By using predefined package structures, you can minimize the amount of writing and possibility for error. Contact customer service for more information.
12. If not using a pre-defined package structure or if any packages are not yet defined on your sales envelope or flyer, indicate the number of prints (not units) for each package as pictured to the right. Add-ons or Bonuses are considered packages.
13. For digital capture only: (13a) Indicate Digital Portrait Option catalog number (from page 78) that describes the enhancement combinations you wish to offer. (13b) Indicate the option type (description) for each category that applies. NOTE: You MUST provide a copy of your sales envelope/flyer with each job. If not included or prearranged with Customer Service, options will be applied to ALL units.
14. Indicate any package inserts for your order. These must be ordered in advance.
15. For “Sorry Slips”, indicate your choice between refund or retake. A “sorry slip” explaining that packages were not printed due to close eyes, etc. will be inserted in each unprintable package and a copy will also be sent to you. ”Refund/Retake” insert should only be used if you plan to return for makeups or retakes.
16. For film capture, indicate how you would like the packages assembled. If nothing is specified, we will individually package and band the packages as shot. If name labels are required, indicate with a check mark and be sure to send camera cards. Be sure sort information is on your camera cards. When name labels are ordered, two purchase lists are included. Packages from digital capture will always include name labels. Default sort is alpha by teacher or grade if not specified.
17. Class Photos & Albums. Be sure to write the catalog number and descriptions of each group, composite and album you order. Check the box corresponding to the product packaging that you wish. A group production form order must accompany your portrait package production form if groups are ordered.
18. If ordering composites in package, submit Composite Work Order form with film/digital files. If composites are deferred with makeups, Excel Photo will generate composite work order based on job data.
19. Fill in the complete shipping address & preferred method of shipping. Be sure to use the street address rather than P.O. Box number. Also, check the shipping method you prefer. If items are shipped to different locations, please indicate this. You may have your film returned to you or keep it filed at Excel Photo for reorders. Please check the box that indicates your preference. CDs will be returned to you when your job ships. If you want to hold delivery of portrait packages for service items, indicate by checking box here.
20. Indicate your method of payment and the amount paid with order.
Attach any necessary student roster lists, package change information or special handling instructions.
